The Office of Campus Accessibility and ADA provide employees with reasonable accommodations in the workplace through the Americans with Disabilities Act. A reasonable accommodation is a modification to the application or hiring process, the position, how the position is performed, or the work environment that allows a person with a disability equal employment opportunities.

Instructions

Employees may request a reasonable accommodation by:

  1. Filling out the information at this link: ADA Accommodation Form
  2. Have your medical provider complete the medical inquiry form located on the University of Kentucky eForms website. The completed medical inquiry form can be returned to the office either by emailing ADA Accommodations or by fax at 859-323-3739.

Please contact the ADA Compliance area if you have any questions. 

All documentation should be submitted within two weeks, but you can contact the Office of Campus Accessibility and ADA if you need a brief extension.
